Oregon Clients

Service Center

The Service Center is available to anyone who is a current CAP client. It is a one-stop shop for community partner referrals, internal CAP referrals, bus fare, and snacks. Clients are also able to access the computer lab, printer, and phone. This is a secure and confidential space. No appointment necessary | Walk-ins are welcomed and encouraged. The Service Center is open during the following hours:

Monday - Thursday 9am - 5:00pm
Fridays Closed, but all staff are working remotely

Peer Support Services

CAP’s certified Peer Support Specialists build relationships with clients based on their shared life experience of living with HIV, mental health challenges and/or substance use challenges. The Peers may assist clients with navigating the health care, mental health care, and drug/alcohol abuse recovery systems, as these are critical to their stability and medical engagement. Here is a list of eligibility criteria:

  • Anyone who is living with HIV or AIDS

  • Completed a CAP intake

  • Income is at or below 300% Federal Poverty Level

  • Currently wanting to connect to mental health and/or substance abuse support

  • Assistance with sustaining mental health and/or substance abuse treatment

  • Currently housed, but struggling to maintain housing

CareLink

CareLink offers short-term case management for individuals that are newly diagnosed with HIV, have been out of HIV medical care for over 6 months, are currently incarcerated or recently released from incarceration and/or are new to the Portland area. CareLink can connect you to HIV medical providers, assist in scheduling and attending medical appointments, discuss medication adherence and make referrals for other services such as mental health support, substance use treatment, housing, and more. Individuals can self-refer by reaching out to the CareLink Team. Service providers can complete this referral form and send to the CareLink Team

African American Service Navigation

In partnership with CAP, Urban League of Portland (ULPDX) provides culturally specific support to African American people living with HIV. The African American HIV Services Navigator is a ULPDX employee with access to workspace and resources at both agencies. The African American Services Navigator can help clients get into and stay in medical care, provide medication adherence support, HIV health education, and link to community resources. Providers can refer clients to service navigation by completing a referral form. Contact Zana Mosley at zmosley@ulpdx.org for more information..

Latine/x Service Navigation

CAP provides culturally specific support for Latine/x people living with HIV to access medical care. The Bilingual Latine/x Services Navigator can help clients get into and stay in medical care, provide medication adherence support, HIV health education, and link to support group and community resources. Providers can refer clients to Latine/x Service Navigation by completing this referral form.

In addition to Services Navigation, CAP offers Reencuentro, a monthly support group in Spanish. This group us currently on hold, however when running, it is held on the 2nd Tuesday of each month. Then Keep it UP is for you!

Keep it UP! ( KIU! ) - is an online, interactive, individual-level safer sex course for young men who have sex with men. It can be used across devices (i.e., phones, tablets, and computers), and uses a variety of content such as videos, interactive animation, and games to increase HIV knowledge, motivate and teach safer behaviors, and instill self-efficacy for enrollees. Oh! Did we mention there is a CASH incentive?

SW Washington Clients

The Service Center at CAP’s Vancouver office is a one-stop shop for housing and basic needs referrals for clients enrolled in Medical Case Management services. Drop by and let us help you find the services you need, or just spend some time using our open computers in a quiet, supportive environment with help available if you need it. Employment Services

ProjectCAN (Career Access Now)- ProjectCAN provides varying levels of support to CAP SW WA clients interested in career exploration, job readiness and increasing self-sufficiency. Monthly educational group workshops are available to all clients with topics such as resume writing, job searching, and mock interviews. One-on-one support is also available for clients wanting to dive deeper into various employment related topics and focusing on finding employment during our Service Center hours:

Peer Navigation Services

Peer Navigation is an individualized level of support available to CAP clients in CAP SW Washington’s Vancouver and Longview offices. Peer Navigators work with clients to develop supportive relationships that are grounded in shared lived experience of living with HIV/AIDS. We also support mental health and substance use challenges. These services are provided in both one-on-one and group settings with an emphasis on reducing social isolation, HIV-related Stigma, medical engagement, medication adherence, and engaging clients lost to care.

We have peer groups that do not require working with a peer one on one. Any CAP client with HIV can join. In Vancouver, on the second and fourth Thursday of the month we hold a zoom peer group and in Longview on the second and fourth Thursday of the month we hold in-person peer groups. From time to time, we also hold in-person events in Vancouver.
